Role Specifics and Benefits: As an LPN in Long-Term Care, you’ll partner with RNs and physicians to implement care plans, monitor resident health, and respond quickly to changes in condition. Core responsibilities include: - Administering medications, performing treatments, and maintaining precise documentation. - Performing vital sign checks, daily assessments, and early identification of clinical changes. - Delivering wound care, skin integrity maintenance, and assistance with activities of daily living. - Supporting mobility and rehab activities, fall prevention, and safety initiatives. - Collaborating with families and interdisciplinary teams to align care with resident preferences and goals, and documenting progress for ongoing planning. - Ensuring infection control, regulatory compliance, and quality improvement practices are followed.
