We are seeking a Nurse Practitioner or Physician Assistant to join
our collaborative Family Medicine team in Plainfield, CT, where you
can grow professionally while managing your own patient panel.
You’ll work alongside 2 physicians and 2 APPs, with a dedicated MA
per provider and a nursing team of three, and benefit from two
nearby offices for easy coverage and work-life balance.
Conveniently located near Providence, RI, and the scenic CT/RI
shoreline, this role offers both professional fulfillment and
access to beautiful weekend escapes. Role: Open to Nurse
Practitioners (APRN) and Physician Assistants (PA) Location:
Plainfield, CT Setting: 100% Outpatient Primary Care – Family
Medicine (11) Schedule: Part-time, weekdays only! Call schedule: No

Assistants Role Scope: Under established guidelines and in
collaboration and/or supervision with the practice physician(s),
Nurse Practitioners and Physician Assistants perform routine
physical examinations, and formulate medical, educational, and
psychosocial plans to treat patients. In consultation with the
physician(s) and care team members, Nurse Practitioners and
Physician Assistants develop plans for further evaluation of health
needs, e.g., referral to other health team members or specialists.
Nurse Practitioners or Physician Assistants provide appropriate
treatment for problems within the scope of the provider’s practice
as designated by protocols and identifies problems outside of their
scope of practice and refers appropriately to physician.
Qualifications Requirements: - Licensure: Unrestricted State of
Connecticut Medical License (or eligible) - Training: APRN (Family,
Adult/Geri Nurse Practitioner) or PA - Experience: 2 years’
experience in a primary care or relevant outpatient setting
