Job Description:
Seeking RN Case Manager for home healthcare in Oswego County
with up to $15,000 sign-on bonus (restrictions apply).
Provides direct patient care, develops care plans, and manages
client caseloads following quality standards.
Conducts home visits Monday-Friday, 8a-4:30p, with one weekend
monthly; no on-call required.
Supervises LPNs and home health aides, ensuring care coordination
and documentation accuracy.
Requires NY RN license, valid driver--------s license, and computer
proficiency; experience in home care, hospice, or palliative care
preferred.
