you! Are you ready to join our team?About Our Bridge ClinicThe
substance use disorder (SUD) Bridge clinic is co-located with
Infectious Disease and provides on-site rapid assessment, treatment
initiation, and stabilization for patients with substance use
disorder, HIV, STI's, and other Infectious Diseases with a specific
focus on equitable care which seeks to eliminate any disparities in
access to or retention in treatment by race, ethnicity, or
language. This Bridge clinic is a novel progressive
forward-thinking clinic staffed by a multidisciplinary team, which
includes a physician, therapist, a peer recovery coach, an advanced
practice provider, and an office-based addiction treatment (OBAT)
nurse care manager. The care model will consist of rapid access to
low threshold evaluation and treatment for individuals identified
with SUD throughout the Merrimack Valley Community. Patients will
be assessed by the team and a patient-centered treatment plan will
be determined. Pharmacotherapy will be initiated promptly for
appropriate patients and patients will be connected to other Bridge
clinic services and Infectious Disease services as needed and based
on patient preferences, including but not limited to viral
hepatitis treatment in the setting of ongoing substance use, harm
reduction services, nursing support, peer recovery support, therapy
and HIV Prep, STT's, LTBI, and Family Planning options. The patient
will be managed by the Bridge Clinic team on a transitional basis
while linkages are made to the suitable and patient-preferred level
of care within the community. Patients who cannot directly be
connected to ongoing addiction treatment services will continue
receiving care at Bridge Clinic until linkage is successfully
achieved. Additionally, the Bridge clinic provider staff will
provide electronic consultation on SUD cases for primary care
providers across the Mass General Brigham System.LocationThe Bridge
and Infectious Disease Clinics are currently located at 500
Merrimac Street in Lawrence, but they will be moving to 600
Primrose Street in Haverhill in August of 2022. General OverviewThe
office-based addiction treatment (OBAT) nurse care manager, in
conjunction with Bridge Clinic and Infectious Disease providers,
helps manage and treat patients with SUD/Infectious Diseases in a
welcoming, low-threshold, and person-centered care model. The OBAT
nurse serves as a care manager partnering with patients to improve
treatment initiation, retention, and long-term clinical outcomes
and may work as part of the SUD Bridge clinic team and/or primary
care settings to support the transition of patients from Bridge
Clinic back to primary care. The OBAT nurse care manager oversees
all aspects of care coordination for patients seen in the Bridge/ID
clinic. The OBAT nurse works with the Bridge Clinic team/ID team
and primary care team to establish and implement protocols for
medical management, substance use disorder treatment and
pharmacotherapy, stabilization in Bridge Clinic, connection to
additional needed SUD treatment or recovery services and linkage to
treatment within the primary care
practice.QualificationsQualifications BSN required, can be waived
with appropriate experience and/or education Current RN licensure
in the state of Massachusetts required Minimum 5 years of clinical
experience with experience in community health and substance use
disorder services preferred Certified Addictions Registered Nurse
(CARN) or willingness to obtain certification within first year of
