Job Description RN Nurse Supervisor (Signing Bonus $10,000.00) Haverhill, Massachusetts RN Nurse Supervisor This is a full-time salaried position primarily responsible for client clinical care and documentation. This position reports to the Director of Nursing. 80% of the time for this position is to be spent outside the office performing client visits and 20% if time is spent in office doing documentation and follow up communications with other healthcare professionals, families and the RAH office staff. Client Care Duties Conducts client/family interviews Perform new client assessments Perform insurance client assessments Supervise the client care plan through follow up visits Communicate with doctors to obtain necessary physician orders and to report changes in the patients medical or mental condition Coordinate with facility discharge planners regarding patients transfer to home care Perform per diem nursing visits, e.g. medication management, medication administration, blood pressure readings, wound care, as assigned by Director of Nursing per clients care plan Report/Investigate any claims of client neglect or abuse Field Case Management Duties Field Case management specific to supervision regarding field staff daily assignment completion of household tasks, maintaining client care supplies, and document management Clinical Supervision Clinical supervision of home health aides Communication with Director of Nursing regarding personnel and/or scheduling issues Makes recommendation to Director of Nursing regarding removal of direct care staff for cause Perform Skills Evaluation - Observe home health aides to create an updated skills checklist Perform internal investigations Investigate of employees regarding neglect or abuse allegations Supervise/manage, field training, skills development, and appraisal of field personnel Meet weekly field client visit metrics Training Perform home health aide and caregiver in-service education Clerical Duties Create client care plan in ClearCare Maintain clinical records in ClearCare Document employee supervision visits in ClearCare Review and file client note narratives Secure signature on contracts, HIPPA agreement, and collect client deposits, if necessary Complete and submit long-term care insurance assessments Document any abuse/neglect claims for Department of Public Health reporting Bilingual encouraged to apply External Duties Represents Right at Home before the public as required by the organization The above noted job description is not intended to describe the multitude of tasks that may be assigned, but rather to give the employee a general sense of the responsibilities and expectations of his/her position. Qualifications: Education and Training: Minimum requirement is a BS from a state approved school of nursing with one year of satisfactory nursing experience in a home care setting. Must maintain a license without encumbrances and be in good standing as a Registered Nurse by the State Board of Nursing. Must maintain a valid drivers license and the use of an insured automobile or access to adequate transportation. Requires familiarity with standard concepts, practices, and procedures used in the home care industry. Requires familiarity with standard concepts, practices, and procedures used in the home care industry; an ability to apply concepts of basic math; and proficient computer skills including electronic mail, record keeping. Skills: Requires self-motivation, initiative, and flexibility; an ability to solve practical problems and interpret a variety of instructions furnished in written, oral, diagram, or schedule form; attention to detail and an ability to prioritize tasks, and handle multiple tasks and projects simultaneously. Requires customer service skills, professional telephone skills, and an ability to communicate proficiently with multiple stakeholders including supervisors, families, employees, referral sources, clients, and co-workers. Requires light physical activity. Relies on limited supervision to plan and accomplish goals to perform a variety of tasks. Must be able to acquire proficiency in ClearCare.
