Motivated team player to coordinate support, supervision and train Adult Foster Care staff nurses; oversee LTSS PA submissions; participate in recruitment efforts; and coordinate medical intake and admissions. Positive and friendly personality tovisit individuals with disabilities and families in Northeastern Massachusetts, ensuring stable, healthy, and fulfilling living environments. To support the Adult Foster Care nurses to provide assessments, education, medical oversight, and referrals regarding personal care in a compassionate and positive home environment; train new department nurses; provide coverage for home visits, as needed and supporting a flexible caseload during periods of rapid program growth. SCHEDULE FOR THIS POSITION: Monday - Friday, 40 hours per week, flexible schedule KEY ACTIVITIES:

• Completion/oversight of medical intake, admission paperwork and case assignments
• Support, train and evaluate staff nurses
• Conduct regular supervisions of staff nurses
• Participate in regular supervisions with Program Director
• Home visits with families and individuals to assess medical status and needs
• Regular documentation of home visits and communication with families
• Scheduling appointments for home visits
• Develop care plans and update as required
• Communication with MassHealth, LTSS and Health Care Providers
• Coordinate other clinical assessment
• Recruitment and administrative support Essential Physical Requirements: The ability to move independently throughout a wide range of environments, some of which may not be wheelchair or otherwise handicapped accessible, is required.   • Current Massachusetts RN Nursing License
  • Bachelor's Degree preferred.
  • Must have three years' recent experience with elderly or disabled adults
  • Bilingual-English/Spanish preferred
  • Valid Driver's License with a minimum of 1 year of driving experience.
